Disease-Management-Programm (DMP)
The Disease Management Program (DMP) is a structured treatment program for chronically ill patients. The aim of the DMP is to improve the quality of life of those affected, prevent complications and ensure optimal medical care. Participation in the DMP is voluntary and offers patients ongoing support and advice.
Our practice offers DMP care for the following conditions:
- Coronary heart disease (CHD): The focus is on the treatment and control of risk factors such as high blood pressure, elevated cholesterol levels and diabetes. Regular examinations, adjusted medication and lifestyle advice help slow the progression of the disease and avoid complications such as heart attacks.
- Chronic obstructive pulmonary disease (COPD): Patients with COPD benefit from close care that includes pulmonary function testing, medication optimization, and respiratory therapy training. The aim is to reduce shortness of breath and increase resilience.
- Diabetes mellitus type 2: The DMP for diabetes focuses on blood sugar control, the prevention of secondary diseases such as kidney damage or cardiovascular problems, and training to diet and exercise. Regular laboratory checks and foot examinations are also included.
- Bronchial asthma: As part of the DMP for asthma, the focus is on the optimal setting of respiratory therapy, the identification of triggers and the prevention of respiratory infections. Patients receive training on the correct use of inhalers and strategies to prevent asthma attacks.
All DMP programs are based on current medical guidelines and are individually tailored to the needs of the patient. Our practice places particular emphasis on a trusting collaboration between doctor and patient in order to achieve the best treatment results.
